Hemosiderin is a hemoglobin-derived pigment appearing as golden-yellow or brown granules. Following phagocytosis of erythrocytes by macrophages, hemoglobin is degraded within lysosomes into iron-free hematoidin and iron-containing hemosiderin. The Prussian Blue reaction is a classical method for hemosiderin detection, in which ferric iron reacts with potassium ferrocyanide under acidic conditions to form an insoluble blue compound. Hemosiderin deposits are commonly observed within macrophages or stromal tissue and primarily represent ferric iron (Fe3+).
In rare cases, iron exists in the reduced ferrous (Fe2+) state. The Lillie method is highly specific for detecting ferrous iron. This staining kit mainly contains potassium ferrocyanide and nuclear fast red. Ferrous iron appears deep blue, while nuclei are counterstained red.
