Lipids include neutral fats, lipoids, and their derivatives, characterized by insolubility in water and solubility in organic solvents. Neutral fats consist of three fatty acid molecules esterified to one glycerol molecule and serve as a major energy storage form. Under normal conditions, lipid droplets are rarely visible in non-adipocytes; excessive intracellular lipid accumulation indicates fatty degeneration.
Sudan Black B staining is used to demonstrate lipid degeneration and abnormal lipoid deposition, particularly in liver, kidney, and heart tissues. Neutral lipid droplets stain dark brown to black. Specimens should not be fixed with ethanol-containing fixatives or embedded in paraffin; frozen or carbon wax sections are recommended. Lipomas, liposarcomas, thecomas, and adrenal cortical adenomas are typically positive, while clear cell renal carcinoma and ovarian fibroma are negative.
